At your initial appointment you will be asked to complete our new patient paperwork. This provides us with your health/condition history as well as demographic and billing information. 

Frequently asked questions

Where can we park? Please use metered parking. There are a number of spaces on 7th and Cordova. Unfortunately we do not have parking spaces available in our parking lot.

What can I expect at my 1st appointment? Our appointments range from 30-90 minutes. Insurance carriers will only cover 60 minutes of therapy. It should be noted that your initial appointment will have a brief consultation prior to treatment.

What should I bring? At your initial visit we request a copy of your driver's license and medical insurance card, if applicable. 

What should I wear? Wear whatever is comfortable for you. If you are requesting Fascial Stretch Therapy please wear athletic clothing as you will stay dressed during your session.

What is required to submit to my insurance? For patients wishing to use insurance, we require a referral from either a Doctor, Osteopath, Chiropractor, Advanced Nurse Practitioner, or Physician Assistant. Referrals must have a valid diagnosis code as well as a duration and frequency for care. All insurance carriers require massage therapists to have a referral for any patient requesting them to bill insurance. 

What is the cancellation policy? We request patients contact our front desk by 12 PM AKST the preceding business day. Appointments canceled by the patient after this time are considered a "late cancellation" and are subject to a recoupment charge of $100.00. Arriving more than 15 minutes late to your appointment's start time will result in the appointment being canceled and considered a "no-show." No-shows are subject to a fee equal to 100% of the visit's total expected charge.
